To register online for treatment, you need to take the following steps:
1) For those who want to start treatment, there is no separate consultation fee. Kindly copy the CONSULTATION FORM and fill it up with your medical details, and mail it to us at info@ayurvedaphysician.com Please send us only a text summary of reports and investigations; do not send scanned reports and images, as these take up a lot of web-space.
2) Those who desire only consultation, may click on the button labeled as “US$15” in the ‘Start Consultation' section of this page. After you pay online the consultation fees of $15, you can access the CONSULTATION FORM from there, and fill up your medical details along with your specific queries. Consultation will be valid for a period of 10 days, in which we will answer 3 specific queries. For our treatment policies, please look up the “Free Online Queries” section.
3) For all chronic problems, we recommend that you order for 3 months’ medicines at a time, since this will help you to clearly define your treatment response, which may not be easily apparent with 1 month’s medicines.
4) The cost of medicines for patients residing in India is as under (Also, kindly refer to point no. 7):

Payment can be made online by credit card (through a secure Payment gateway of PayPal); or by cheque/DD made out in the name of Dr. A. A. Mundewadi, and posted to the address given in the “Contact Us’ section. For local patients who visit our clinic (after taking prior appointment), there is an additional nominal consultation fee of Rupees 500/- only.
5) The cost of medicines for patients residing outside India is as under (Also, kindly refer to point no. 7):

Payment can be made online by credit card (through a secure Payment Gateway of PayPal); you can also pay directly to PayPal in the currency of your country, making the payment to the e-mail i.d. drmundewadi@yahoo.com
Kindly note that the difference in Indian and International rates is due to the following two reasons: 1) The treatment packages are different, and 2) We do not provide a prescription for local consignments up to the national level. Indian nationals residing abroad, who try to procure medicines at Indian rates by submitting local addresses, should note these points. In addition, international clients should note that the prescription is provided so as to meet international shipping requirements, and to avoid delays in Customs and security checks. Please do not misuse the prescriptions for self-medication. All patients require regular follow-up and periodical assessments, with appropriate and timely treatment modifications, so as to get the maximum possible benefit from treatment.
6) Shipping for local consignments is currently free; however, we reserve the right to levy courier charges in future, if required. For international consignments, you will have to pay additional shipping costs of $30 for SpeedPost (Express Postal Delivery) or $50 for Private Courier. These charges are subject to change according to current applicable rates.
7) Please note that the above mentioned treatment costs are baseline costs. Actual treatment costs vary considerably from patient to patient, depending upon the severity of the disease, and the associated complications. Kindly check with us before sending in your payment.
8) International clients who wish to pay by Western Union Money Transfer can make payment in the name of Dr. AbdulMubeen A. Mundewadi, payable at THANE, INDIA. After making your payment, please send us the following details: 1) The MTCN number 2) Name of the person making the payment 3) Place from where payment made 4) Specify the amount and currency.
9) For online payments, currently it is possible to make payments for the following amounts: $60, $80, $150, $200 and $15 (the payment button in the “Start Consultation” section). Please use a combination of these payment buttons to make your payment. For example, if you have been asked to pay $350 for medicines and $15 for SpeedPost, click once on the $200 button, once on the $150 button, and once on the $15 Consultation button.The payment buttons have been fixed to automatically add shipping costs. If you have been given a treatment cost-estimate which includes shipping costs, please complete your credit card payment; we will refund any extra amount that you have paid.
10) Patients desiring treatment for HIV/AIDS, cancer, kidney failure, heart disease, and other diseases with a known serious or fatal outcome will need to sign a “Consent Form” before commencing treatment.
11) Once your payment is confirmed, we will dispatch your medicines to you.
12) Make sure that you have given a detailed and correct address for delivery of medicines.
13) You will have to pay any additional taxes/ duty etc. as applicable in your country.
14) After dispatch of medicines (about which we shall inform you) you should get the medicines within 2 – 3 days in India, and within 5 – 20 days outside India.
15) We accept no responsibility for delays occurring due to the import policies and customs regulations of different countries.
16) An order once placed, cannot be cancelled under any circumstances. There is no money back or refund policy.
